Different strengths of Alprostidal?

Postby Martin25 » Mon Sep 19, 2022 8:42 am

Can someone please tell me what the different strengths of Alprostidal are? I was led to believe by my doctor and his compounding pharmacy that there are only two, the standard strength and double strength. Is this correct? I ask because I have yet to have an effective result with injections(although I reached around 70%), and my doctor gave me the lowest dose of tri-mix which did not work.

Dr Eid, Who is a leading surgeon for implants, is not a big fan of penile injections, especially using tri-mix… I am considering seeing a different urologist who is willing to work with me more closely to see if I can get results from injections. Can anyone recommend a urologist in the new York city area? If I need to go down the implant Road, I know Dr. Eid is an excellent choice, but I feel he has been so conservative with injections that I am not giving this route a true chance. It has been four months with him with only incremental progress and I have to see him for an office visit each time for a change…

Any referrals you can make are appreciated…
63 years old, pills stopped working; Struggled with injections and restriction bands for past 2 years, minimal success. I’ve finally come to terms with needing an implant was scheduled for November 1, 2024, but my surgeon is on sick leave.

PS I was prescribed 40PGE…Dr Eid injected two full syringes, right and left side of penis, which he said would equal 80PGE, full syringe…response was 75% erect…is 80PGE the maximum strength? I felt I was close..

That may be the only options offered at that pharmacy. At a compounding pharmacy that does their own mixes the alprostadil mix is whatever they want it to be. My compounder (Florida only) will mix whatever the doctor orders. My mix is currently 80mcg alprostadil per ml. Some compounders resell mixes they get in from a major compounder and will only sell the mixes offered to them. For consistency or simplicity and/or ease of compounding, book keeping (or other reasons) some compounders only sell certain set mixes possibly decided on in consultation with doctors.

Yeah, from what I hear Eid doesn't like tri-mix, possibly with good reason.

Can't help you there.

I'm not sure I'm clear on the dose the doctor gave but if he gave you 2 full milliliters of 40mcg/ml at once then that is a lot. Not the most I've heard of but it really is a lot. If you previously mentioned the cause of your ED I don't recall. For instance, if you're a recent prostatectomy surgery patient and possibly still recovering (which can take a couple of years) then injections could still work for you down the road a bit. Otherwise injections might not be the best option for you.

Not a clue on that.

The cause of my ED is a mystery, I have not had any surgery. I am at 61 year-old man who is in pretty good shape, not overweight and I don’t smoke and I’ve had ED for years now, but it responded to pills for a while; the last 2 to 3 years nothing has worked.

I just heard back from Dr. Eid’s office and his receptionist/nurse said he agreed to prescribe tri mix ‘full strength’, whatever that is, and to begin by injecting 50, and to titrate up 10 units at a time. Does that sound reasonable? I’m wondering whether I should not go into his office to inject the first time, to better gauge my response and in case I run into problems…?
I wish the alprostidal would work for me as it has worked for you, but I am feeling a bit desperate at this point so I am willing to try the tri-mix.

If there are any tri-mix users out there who would care to weigh in, please do so… I thank you in advance.

John: saw your post on alprostadil. Been there done that I think you should speak to an expert about implants. I went through all the steps with injections, bi mix, tri mix various conentrations, various injetion sites, etc. Both Maureen and I got really frustrated as sometimes it would work for ten minutes, others pushing the magical four hours, and others just nothing.
I went in and talked with my uro, who was also my prostate surgeon and he advised an implant. My first reaction was they are not doing that to me!!!! After a particularly frustrating session, I said to Maureen I am going to go for it. WE did. Fortunately, she was with me the whole way and we both love our implant. If you and your wife aould like to discuss it, we are cool wtih that including show and tell
Frank
I recommend you also talk with Dave. He and Jeanene are aslo very pleased wiht theirs. We both have Coloplast Titans ltho I donlt think the brand is all lhat critical/. We could do facetime or zoom

PS I am sending Dave a copy of this
