Titan Premature Failure 2y > Infection from Revision > Tactra Salvage and length loss

Hi friends,

Had my revision done yesterday at 07:00. First day was great, got out of the hospital same day at 20:00.

Started having moderate pulsating pain after I got home, similar to the first surgery, precisely located near the new pump.
Of course, peeing was also very painful after I got back home and after the anesthesia post effects subsided, and I almosted passed out trying to empty the bladder for the first time at home. I woke up from surgery without a catheter this time.

The culprit for this revision was a tubing failure near the pump. I have attached a photo from the surgery showing the tiny little hole that caused this failure. It's hard to believe that I had to get opened up to fix this tiny little fucker of a rupture :lol:

The whole system was replaced except for the reservoir. At my request, we did get it topped up to 140ml since I was easily maxing out the previous amount of fluid (we suspect it was 90 ml from the first surgery).

Relatively rought first 24 hours as expected, I'll be laying down as much as I possibly can.

I'll try to provide some updates for my own referencing in the future, hopefully some information is also useful to any of you going through something similar.

I guess they haven't designed a 'patch' for tubes with such a small breach? Seems I've seen on youtube surgeons cutting the tubes to the proper length and then using a connector to make a leakproof connection. Sure seems like it'd be far less traumatic to install a patch coupler there rather than go through a complete replacement.

I suppose since they're in there they might as well restart the clock on the whole implant, but boy that'd be tempting to just patch and be back on your feet in a day or two.

Surgeon said the exact thing, however I did want to get an upgrade to the XL size since I wanted to reduce RTE's. The only thing that was not replaced was the reservoir. It took me a long while to feel it was completed healed. It'd bother me even nearly a year after the initial surgery after I was running or jumping for more than 20m. The end of 5ks was painful due to the sharp pain near the reservoir. That just happened during heavy exercising though, daily routine was completely uneventful, so I also requested to maintain the current reservoir unless there was a real reason to have it replaced.

It has been stated by a number of sources that the tubing is a less than strong part of the Coloplast device. I have read that Coloplast has the tubing made by another manufacturer. Unlike AMS that produces tubing inhouse. Probably best you had all that old tubing removed and replaced.

I'm thinking the tubing was damaged by the forceps. In the YouTube videos, it seems that the tubing is frequently pinched shut as all the parts are assembled. They don't want any saline to leak out of the reservoir.
Not saying that this is common or a large fault of the surgeon. But I'm betting on that scenario .

Interesting hypothesis. We'll never know however.

Thanks. Recovery is already much better than the first surgery. Currently, I have no pain.

Would love to know the difference you feel going from standard to "XL"
Im 22 +1rte and was disappointed to not have the 24xl
56, ED 10-15 years. Pills, Bi, Tri then Quad Mix Injections- all slowly less effective. IPP July 2022 (Penscrotal, Titan, 22 + 1) @6mo back to OEM--7.2"L x 5.5"w
