I hope you all are doing great in your life with your implants and wish for the best for all of you brothers. I have been researching on implants in this forum for a couple of months and feel very good when you guys help each other.
I have a question can you guys help me out? If somehow infection occurs in IPP then what will happen? In some cases they put a malleable which is fine as patient can resume sex, but why in some cases they leave nothing in penis and after some months they again try for IPP? Does this depends on patients choice?

The only choice the patient has is if he still wants to try to get an implant another try.
If so then a lot depends on how fast the infection is discovered and treated. There was one case on this board where the patient insisted on treating the infection with more medication and the infection did resolve and no surgery was needed nor done. This is the only case that I know of. There were other videos online where a very good doctor would remove the infected device and do a wash out and place another inflatable device. Those are cases that are caught early and very good surgeons. Not many of those are done. Then I hope the most common is that a malleable is placed to prevent a significant amount of size loss. With the malleable there is still a bit of size loss. A really good doctor can place a malleable in almost all cases of infection. The placement of the malleable is to prevent size loss. If the doctor is just average or a bit below nothing will be placed. And a large amount of size loss will occur.

If you have visible signs of infection/erosion IN the shaft or glans, your implant will have to be removed, and you will need to come back after 3 month's healing period with reduced girth and reduced length for any new implant. Your glans or shaft has to heal first after the infection in order to even accept a salvage malleable.
If the infection is possibly NOT in the shaft or glans, your implant will still have to be removed and you can get a salvage malleable in the operating room to preserve your length and girth. You can either keep it or come back again after 3 months for an inflatable implant.
Some inflatable experts will be fine with replacing with an inflatable while removing and it all depends on his/her judgement, expertise and also the severity of the infection.
A lot depends on the surgeon's judgement in the operating room.

I know it obvious but, with a really good Dr the infection rate is 2-3% even less in some cases. There’s lots of other issues you could face that’s in the same percentage of risk that we never consider. Guess I’m just saying do your due diligence on cleanliness before and after surgery and don’t worry about it. Easy to say, hard to do I know but once I had mine installed I don’t think I worried about infection. Was more concerned with getting healed up and past the discomfort stage so I could use the thing! And it’s been a game changer!!! 26 months and going strong!
59 years old ED started mid 40s pills failed after 10 years. Injections works but diminishing results with pain. Implanted 5-22 Baylor,Scott,and White Dallas.Dr Michael Wierschem, infrapubic Coloplast 20cm and 1cm RTE. Going strong and loving it!

You may want to look at Dr. Eid’s website, he has a link to you tube videos of his, and I think one has to do with infections.

Is the reduced girth and length a given? There's a post where one guy had nothing in there for a year and didn't seem to have the issue.

I think it is a good idea to have an infection plan strategized with your doctor before the surgery. If you can't get a urologist who does implants to manage the infection the device will be taken out with no replacement and a patient will suffer a lot of scarring and tremendous size loss.
From the information I have read I am under the impression that a lot of men lose around half their size. And then there are men who can't have a replacement at all of either a malleable or an inflatable and are left with nothing.

Born 6/15/74. I have substantial venous leak with fairly severe hour-glassing, but no hard plaques. My urologist is sexual health expert Dr. Laurence Levine who performed a Doppler Ultrasound and diagnosed me with VL in 2020. I also have mild BPH

Maybe not, but Eid warns about it in a video and he is the world's most recognized expert on implants.
Also, I read a post from a guy who had nothing for around the same amount of time and suffered too much scarring and closure to put anything back inside.

If excessive scarring prevents the installation of an implant, a consult with a reconstructive urologist may be an alternative path to explore.

I just lived this subject.
Very minor infection with a bit of drainage from my incision line 6 weeks after my initial implant placement. 3 weeks of antibiotics nearly cleared it up but Dr. Clavell was firm that the implant required replacement. He is a credit to his profession and responded with kindness, support and confidence.
Replacement Titan surgery was simple with an easy recovery, much easier than the initial placement.
Now post-op 8 weeks with no loss of size and thrilled with my results. My wife and I enjoy a sex life like we did 30 years ago.
Moral of the story: Seek out an experienced expert in the field and trust your instincts in evaluating their bedside manner. Dr. Clavell has earned my forever trust. And I'm a surgeon.
