What’s the best malleable that gives maximum size?

Regicon
Tactra
Zephyr
Promedon tube
Rigicon MALLEABLE
20 cm + 0.5cm RTE
11 mm diameter rods
——
Pre-Op penile length: 7.9 inches length, 4.75 inches girth
Post-Op penile length: 7.5 inches length, 4.5 inches girth

The Indian Shah malleables have the largest girth in the world, with each rod measuring 15mm in diameter, but this model is only available in India.

If you're looking for the largest girth outside of India, the Rigicon Rigi10 with a 14mm diameter rod is the best option.

However, if GIRTH is your PRIMARY concern, I would recommend considering inflatables, as you might find malleables disappointing in this regard.

I went to Ashish and didn’t like him. I’m doing it with Nagercoil. I’m honestly stuck between a Promedon tube and Tactra. The regicon representative was just retarded and it put me off the brand.

If the sizing and placement is incorrect, even a thinner rod can lead to erosion. It's important to leave the technical aspects, like sizing, to the surgeon. As indicated in a study, the best size for you, is the best fitting, neither the oversize nor the undersize. If 14mm is the best fitting for your anatomy, a model like Rigi10 is probably the way to go.

Promedon has an 18 year real-world track record and it's hard to ignore that.

Both Tactra and Promedon were measured for an impressive axial rigidity of 3 lbs per rod.

Tactra's Nitinol core is also tested for 100,000 bend cycles. Looks like Tactra also has an edge!

Promedon isn't used in the United States and it makes me wonder why

And what you said about the sizing of the rods is correct, but some guys push their surgeons to maximize size and surgeons will often comply.

It reminds me of that study where the optimal rod diameter was 11mm for most guys and I'm sure many of the guys would want thicker rods, but 11mm was optimal for avoiding complications
Born 6/15/74. I have substantial venous leak with fairly severe hour-glassing, but no hard plaques. My urologist is Dr. Laurence Levine who performed a Doppler Ultrasound and diagnosed me with VL in 2020. Surgery scheduled for Coloplast Genesis 5/19/25

Yes. Surgeons do comply.

Sizing challenges seem to be a red flag with Promedon.

"Common postoperative complaints were: prosthesis too short in 27 cases (32.5%)....."

I’m thinking to keep it simple and get a Tactra. It seems like the best implant. If I do lose a lot of girth, I guess fat injections are always an option , but honestly a little bit of girth loss vs finally resolving ED

The study you're referring to is based on outcomes observed at one high volume center. The principal investigator of the study is from Al-Themal Medical Center in Abha, Saudi Arabia.

I wonder if the complication rates and sizing correlations noted in the study are specific to the Middle Eastern population and anatomy. Whether these findings would apply to the U.S., Europe, or other parts of the world is uncertain.

Perhaps the rod diameter and complication correlation question can only be answered through a study that represents different anatomical diversities of other populations.
