So, got a prescription from urologist who suggested we start with Alprostadil first before considering Trimix. Test shot at clinic was 15 units which is quite moderate and it resulted in a very slight chubby.
Got sent home and advised to increase by 5 units till sufficient response.
So far I've had 4 attempts going as far as 40 units today and no effect at all. I don't think there's any way I could possibly have missed the spot on these 4 attempts.
Could this mean that this just won't work for me and I should try Trimix?

I have been using trimix only about a year so I don't have the experience this some other guys have had but it has worked great for me.
In the past 12 months I have injected about 77 times and have have had only 3 or 4 misfires.It only takes about 5 or 6 minutes to achieve an erection after injection.

If there is no effect, you are likely not injecting right. Alprostadil is very effective if injection technique is correct, and it is safer than trimix, which can cause scarring. Please consider going back to your doctor office and doing the injection there under medical guidance. It is not so easy for everyone as it may seem. We all have different anatomy. I struggle with getting into the correct depth myself because I have very dense tissue surrounding the corpora, which will bend the needle instead if letting it through to the correct depth.

Also as a follow-up to my earlier comment you may consider getting a auto injector called inject -ease.For me it makes injecting easier and more precise in putting the needle exactly where you want it

How do you get around the dense tissue issue? I may have that as well.

I've actually tried twice with the auto injector. On both attempts, for some reason, the medication failed to go into the corpora as if it kept hitting resistance.

Reading your comments, I'm curious as to the root-cause of "scarring" of the penile tissue. What is it about alpostradil versus trimix that overcomes the needle/injection related issues?

Dr. Eid told me that papaverine (one of the ingredients of bimix, trimix, and quadmix) causes fibrosis. There were lawsuits about this back in the day. He said alprostadil alone doesn't cause fibrosis.
But in my case I think a lot of the scarring is simply from needle damage over the years, like what happens to junkies' arm veins, even though our medication is legally prescribed.

Please read as many of the old injection threads as you can stand. To me the most importand determination of proper needle placement is plunger resistance. If the needle is in the correct spot the plunger will be easy to depress. If it doesn't depress easily you aren't at the correct spot. Try a little deeper and if that doesn't help try a little shallower.
I will point out that the smaller the capacity of the syringe the easier the plunger is to depress. Also some brands have plungers that are easier to depress than others. My experience is that the otherwise high quality "BD" brand has difficult to depress plungers. The easier the plunger is to depress on it's own then the easier it is to determine proper needle placement by plunger resistance.
I'll also note that if the syringe is prefilled for more than a couple of minutes be sure to leave a little air at the top so that immediately before injection you can push the air out and by doing so move the plunger from it's resting spot. The plunger seems to take a set after sitting and be a little difficult to get that plunger to make that first little budge. That can throw off the ability to determine when you have proper needle placement by plunger resistance.
