Hello gentlemen
I would like to share my case with you (I would like to point out in advance that I am under the care of a urologist). My erection is satisfactory only when the penis is manipulated, a moment later it falls down, changes in position weaken the erection, it is strongest in the standing position, on one side, and also in the missionary position. On the other hand, the erection when I lie down is weak and when I sit, the penis shortens significantly but the head becomes much harder. At the same time, I will add that he responds to Cialis or Viagra (it was better, but it is getting worse). You can see an improvement in erection after visiting a physiotherapist and relaxation of the muscles around the spine and pelvis
I am waiting for your opinions, I have some thoughts and I do not want to reveal my thoughts because it may disturb the view and final conclusions and finally, maybe it should have been mentioned at the beginning but I am 29 years old, 180 cm tall, 85 kg in weight, I have a posture defect, anterior pelvic tilt.
unstable erection
Re: unstable erection
looks like vasculogenic issue.
there's surgery, if you believe in embolization, there's maintanance with peptides and pde5's, there's implant, there are new procedures like the regeneration, but its not gonna happen anytime soon imho

Sounds like you’re in the right path. I’m fixing anterior pelvic tilt too. Might try acupuncture or blood cupping as well.
It’s weird how the younger generation is so affected by issues like this. So common.

Re: unstable erection
I had this same problem. I was a 57yo, very healthy, fit man with long-standing ED before having my implant. My erections then were so inconsistent; literally, an absolutely rock-hard and throbbing hard-on one second, that completely melted the next. No amount of peptides, Viagra/Levitra, Bi/Tri/Quadmix, VED pumping, or cock rings made any lasting difference. Trimix at escalating doses got me maybe 15-20 soft erections before it completely fizzled out. Best of luck to you

Were you able to get rock hard without stimulation, the time when it was inconsistent?
