Hi frank, I agree. There is one reason they don’t promote malleable : they know the patient won’t ever come back and he’ll be satisfied first time around. I faced a similar situation with tattoos, I like dark bold tattoos and the artist in England convinced me to get a realisiric one which I hate and it needs to be touched up. He knows dark skinned people ,, shouldn’t get realistic tattoos but he knows touch ups and dissatisfaction and cover up = more money,,, this is my analogy I thought of.


Great example with tattoos. I heard from a prosthetic surgeon who works closely with implant engineers that the more natural or realistic a prosthesis is designed to be, the less reliable it will be. The best is not always the best, depending on how it's viewed.

Very true! No one wants to be stranded without. Working penis. Imagine if a prosthetic in your leg could just malfunction and you’d randomly become unable to walk. That prosthetic would never see the market.

If insurance covers or there's plenty of money to burn or invest, one can get an inflatable, cycle it consistently for a year to maximize tissue expansion, achieve the benefits of increased girth and expanded surgical capsule.

Over time, when it inevitably breaks down (as it surely will), switch to an aggressively sized malleable with the largest available diameter, maybe that loss will get compensated to some extent.

What I don't like about this approach is, each time we undergo a revision, we are once again exposed to the risk of infection and potential size loss, if the explanted device it not salvageable, depending on the severity of the infection.

End of the day, it's all about consenting to some form of risk-benefit ratio game.
