I just got a replacement done by Dr Hakky. The pump is sitting too high behind the balls and the pump seems to be a little horizontal. It’s been almost 4 days post surgery. I tried to pull it down but it barely moves.
How long did it take for you guys to have the pump sit in the right location. And what’s the best location for the pump to be in? Right now mine is too high behind the balls and thus very uncomfortable.

A young guy in his early 30s from South Asia. Implanted Titan 22cm by Dr. Tariq Hakky in Atlanta (Dec 2020). Amazing doctor! No pain during recovery. My size is 5.75” x 4.6” which is better than pre-OP size of 5.6” x 4.5”. Hoping to gain more in future.

Unfortunately not. I am following the doctor's recommendation of pulling it down 3 times a day for 30 seconds each. But it isn't moving much. It goes back way up behind the balls once I sit down or walk a little bit. I am trying to be patient and hoping that it comes down and settles at the bottom of the scrotum eventually. I will know for sure a month from now.

Damn was hoping the news would be happier after 10 days, I emailed my surgeon who said exactly the same that it should be abled to be "pulled" into position overtime, the time is trying to grab hold of the pump is the current location is like trying to grab a 3d object in a 2d space really hard to get any kind of grip on it. I can at best try and nudge it but doesnt seem to have much of an effect.

I'm intrigued by the reports that men with the penoscrotal approach were advised to pull down on their pump while healing. I've only heard that with the infrapubic approach, as the doctor is unable to place the pump at the optimal location due to the access point, so they need the patient's help while recovering.
I had a penoscrotal installation, and prior to the surgery Dr. Clavell confirmed with me that I am right hand dominant. He said he'd place the pump low in my scrotum behind the boys with a slight rotation to the left (looking down at it) so that my thumb would fall on the button naturally without having to pronate my wrist. I assume he would have rotated it the other way if I was a leftie.
That's exactly what I ended up with and was never told to pull down on it or otherwise disturb it while healing. I'm not clear on why a surgeon that has complete access to the scrotum through the incision would need the patient to pull down on it to get it situated properly, seems to me that's their job.

2/22/23 AMS 700 CX 21cm + 1.5cm RTEs. was 58 yrs old, wife of 37 yrs. Penoscrotal. 100ml Conceal reservoir. Dr. Clavell. Pills failing and went right to implant. 18 mos post op: 7 3/4" x 5 7/8", now 60yrs old in 2025

61 years old,T2 since 2008, ED since 2010. Pills have stopped working and not interested in shots. Implanted 11/20/2024 by Dr. Calvell with AMS 700 CX 21cm+1cm RTE

Very interesting! It seems Clavell is pretty good at placing the pump. My personal experience is that yes pulling down on pump is recommended by most urologists who do penoscrotal surgery. I had to do it 4 years ago during the first implant too. This time for some reason the pump feels way higher than what used to be before but I remember that when I got my first implant, I was instructed to pull down then too and it eventually settled down at the bottom of my scrotum after a few months. I hope the pump goes down a bit with time. Hakky did tell me that since I have classic genesis pump, it’s small and can be adjusted to pretty much anywhere where I feel comfortable.

How low is it behind the boys in the scrotum? Mine is behind the boys too, oriented slightly to the right which makes it easy to pump and deflate. My only issue right now is that the pump is higher behind the boys. Dr Hakky told me to pull it down and it will settle in the right place, so I am doing that and hoping it all works out in a few months from now.
Would it be possible to share a photo of where your pump is with me via DM?

@indusvalley- a picture would do no good, as you can't see the pump or it's location in my scrotum from the outside. Unfortunately, you would merely be a recipient of a picture of some random guy's scrotum.
The way I know it's lower is if I cup by own balls from the bottom of my sack and lift up, the first thing I feel is the pump bulb, and then my testicles. Also, sometimes I have to move a testicle out of the way to get to it, so I know the pump is low and behind them, which is what I wanted prior to surgery.
