There are a couple of things someone could try before going to an implant, especially if they are younger and may be suffering from psychogenic impotence. To the person who had a prolonged erection requiring drainage from an alprostadil injection, reducing the alprostadil dose or changing to bimix would probably help. Some urologists in the US provide patients with phenylephrine to inject to end the erection if it lasts too long. Unfortunately, from what I've read on these boards, bimix seems to be hard to get outside of the US. An alternative to intracavernous injections is intraurethral alprostadil gel. In the US, this can be ordered from several compounding pharmacies. It doesn't work for everyone, but if it does, it avoids needles, which may reduce the risk of damage and scarring of the cavernosa. Outside the US, a cream containing alprostadil called Vitaros is available. This is applied to the opening of the urethra and glans.

Many pharmacy chains sell cheap generic versions of tadalafil from fly-by-night Indian and Chinese manufacturers nobody has ever heard of, some of whom have received multiple FDA warning letters for bad chemistry, carcinogen content in pills, and unhygienic manufacturing practices (see for example Torrent). The FDA won't even inspect ED meds because they are not considered essential medications. There are very highly respected Indian manufacturers too, but their products are seldom found in the U.S.
