Cigna here and was covered though it fell under gender reassignment.
No I did not have reassignment but the doc made a case and they came back with Ok.

No kidding. I wonder if I could have my costs reimbursed through Cigna. Though I doubt it'll happen. Is there anything like an appeals process?

You could try but I think they would be hard pressed to pay it retroactively.
