Hi:
Your post is a little confusing. You say you have atrophy of the cavernosa ("almost like a thread"?) but that a doppler also revealed no issue. Have you gotten any kind of solid confirmation, aside from just some doctor's unverified opinion, that you indeed have atrophy/fibrosis?
Your problem sounds like it could be similar to what I was experiencing, which was a tight, contracted corpora which came on following a bacterial infection (I suspect an injury could do the same); it caused "hourglassing" and strange bending in the penis. I too was apprehensive about the implant, but it's so far solved all my issues; the doctor even confirmed that I had great blood flow and no corporeal fibrosis (which shocked me after four years of the tight corpora causing impaired blood flow, lack of nocturnal erections).
But, please, DO NOT just follow suit. First exhaust every other possible issue which could be causing your ED.

You got it wrong man. Even I have lost 1.5 inch due to penile atrophy but my Doppler test says I am fine.
Oh! I see your problem started with a hair loss drug, so something similar to "Post-Finasteride Syndrome." I haven't heard of anyone experiencing penile shrinkage from PFS, just erectile dysfunction; but both may operate along the same mechanism by blocking DHT, which is an androgen required for male sexual functioning and also penile maintenance. I would go to your doctor and have a full hormone panel done.
I wish we could call someday we both had hard flaccid. No idea what platform to contact you on.
