Doc suggested skipping injections, going to IPP

Late 50’s gay guy with ED from BPH, meds for high blood pressure, adhd, depression (have tried different combinations & different medications). Have been on PRN & daily viagra/cialis combinations with limited effects. Can get erections but usually not enough for anal penetration or if able to penetrate can’t sustain the erection. Sex is very anxiety ridden if I’m trying to top, which I would love to be able to do. Saw new urologist who is a high volume surgeon who suggested IPP, which I did not even know was an option. Apparently, my insurance has started approving them. The first application was rejected, but they are filing an appeal, so I feel like it will probably go through upon appeal. Otherwise, he would not have suggested it. Am I crazy to jump straight to an implant if I’m able to? Spontaneity is very important to me so that’s the reason I would like to skip the shots.

I never tried injections because they weren't covered by my insurance.

But I also either couldn't get or maintain a hard on. Anxiety was a constant companion for years for me.

I have no regrets about skipping injections and going straight to the implant.

I have done injections for over a decade. If I had to do it over again, I'd have gone straight to the implant. There are just a lot of downsides to injections. Doubly true if you're single.

You and I are pretty similar. 56yo gay guy.

I’m married to a guy with an open marriage so sex is both at home and out and about.

My performance started diminishing in my late 30s. I finally recognized it and asked my doc when I turned 40. Starting with pills for about 5 years, then shots for about 10 years.

When they worked they were great. But it’s hard to be spontaneous with pills and shots. Some guy giving you the eye at the gym? Want to go home with him and top him? Do you have pills with you? Do you take them now? Do you wait? Use shots? You probably don’t have a shot with you so you can’t top him…. Lots of missed opportunities.

On the other side if you are going on a date or going to see a fuck buddy you come prepared and it can be OK. If you are married and sex is always at home it can be OK.

When the shots work well they are great. The injection itself isn’t much worse than a mosquito bite. I got good at injecting so I never worried about missing the spot. They were very dependable.


But they became less effective and I got an implant 6 weeks ago. I love it. I’m as hard as I want to be whenever I want. It’s the very definition of spontaneity. Your hard cock can become a super power. With a shot or a pill you fuck when the medication is working and are then done for the day. With an implant you can fuck. Cum. Keep fucking. Doze off. Fuck again. Wake in the middle of the night. Fuck. Wake in the morning. Fuck again.

If your sex is predictable, at home, and from a practical standpoint you don’t expect to have sex more than every other day, pills and shots are a good option. But for spontaneity and more frequent sex an implant is king.

I was on the fence for a long time because I thought I would have to trade my organic erections for an implant. I liked my (shitty) hardons. But it turns out I still get a stirring when I get turned on. I still get engorged. I still wake with throbbing morning wood around the implant.

So for me it is the best of both worlds.


It’s a completely legitimate decision to skip shots. Just consider your situation and if they are a good fit.

Ryanryanryan, I tried shots about a dozen times and they didn't work well even at strong doses -- so I didn't get the full benefit. That said, I could tell they would not be an ideal solution for me. In contrast, now that I'm 90% recovered, I can tell that the implant will be what another forum member called "the best of both worlds". The sensation and engorgement are good, the pump up is only a minute to minute and a half -- so even if you had to conceal that it wouldn't be an issue. The complete control of an implant makes it worry free and let's me focus on all the good stuff.

Ryan3, I’d concur with the other guys here. I went to injections about 5 years ago after the reliability of oral meds required dosage levels that weren’t tenable- headaches, flushed feeling, “Viagra hangover”, etc. Shots were awesome at first but over time caused fibrosis and curvature. So I stopped and am planning IPP surgery for late summer. Wish I had gone straight to that 5 years ago. Good luck!
RP 2012, fought ED with viagra and cialis for years then switched to trimix for 3 years. Then scar tissue settled in. Now fighting atrophy with a VED and getting ready for an IPP in Oct 2025.
