Surgery notes/report

What is the proper document one should ask for relating to observations and work of surgery? Is it referred to as "surgery notes"? Anyone know the proper office to call for the request (OR, customer service, etc??)? And, since my surgery was on 3/11/14, would it be to late to make such a request?

Just call the hospital, tell them what you want, and you will be directed to the proper department.

Mine was called a Surgery Report or some such thing. Different hospitals probably have different names for what these reports are called. No, it's not too late to request this, as the hospital probably keeps this information for years.
I'm 75 and got an AMS Ultrex in 1995. Had it replaced in 1999. Got a new AMS 700 LGX in 2013.

Had my surgery on April 17/14 and at my 6 week visit with the surgeon requested the same from my surgeon - just received it this week in the mail. The title on it is "Operative Report"
Radical Prostatectomy Aug 1999 (age 48); Implant AMS 700 CX, Apr 2014, Dr. Brock, London, Ontario

Thanks Guys, Got them on the way. It was no problem, nurse at surgeon's office quickly informed me he would fax or mail.
