Baltimore is great! Guess it'll be Andrew Kramer, right? You guys got the most excellent surgeons at hand, and reading names like Milam (Nashville), Morey (Dallas), Eid (NYC) and Kramer is turning me jealous.
In my humble opinion a flight might be feasible 1 week post-op, so better stay in a hotel meanwhile. I don't recommend putting ice on the swollen scrotum in case of a penoscrotal incision. Coldness impedes the physiological process of wound healing. 1 week post-op walking should be feasible, and walking brings more detumescence then ice packs.

First off, I would ask your Dr or his staff what they recommend. Many of the better docs have many patients that fly or otherwise travel a substantial distance, and they will know as well as anyone what works, and more importantly, what they want you to do. My doc's staff was pretty adamant that I stick around a few days even if I felt ok. Among other things they wanted me resting on my back to minimize swelling, even if I felt ok (3 15 minute walks per day only, at first).

I had surgery on a Wednesday and flew home 3 days later on a Saturday. A more detailed account is in the link in the signature. Dr Eid did tell me of a patient of his who had surgery in the morning and flew home (NYC to Ohio) the same afternoon. NE_Bob (use the advanced search function to see his accounts) had surgery one day, went out to dinner the same night, took a cab to Grand Central Station the next day, took a 3 hour train ride home, then did house work. So, these are the easy cases. Other guys can barely move even weeks after.

The advice to just take it day by day is sound.
