Hi Seeking, Echegollen, Oxford, C_Lab and the rest of you,
Met with Dr Herwig in Vienna today who conducted the Cavonogram CT scan. Only took 10 minutes with the new CT scanning technology which wasn't around when I had my first cavo prior to ligation surgery back in 1997. He told me I had a massive V leak deep my penis which from the glass half full perspective means I can have the surgery tomorrow albeit they may have to cut into me twice to get at it which isn't so positive! Said the surface leak I.e. deep dorsal vein would have been dealt with during my ligation previous ligation surgery but the deep persistent leak wouldn't have been touched because of lack of knowledge of the anatomy back then which as I'm told is because the scanning technology wasn't as it is today.
Praying it all goes well tomorrow and that my 'Massive' leak can be traced back to my pelvis so it can be shut down.
I'll report back on Wednesday when I'm back in the UK.

42 years old, Venous leak all my life. Pills worked but not so much then I foolishly did Scerothreapy (See young guys thread). Now totally impotent and just want an implant to stop me from completely crazy.

I'm 39 years old. Never was able to maintain my erections for more than 1 minute. Pills don't work. Had sclerotherapy by Dr. Kuehhas in Austria in 2016. Didn't work. Injections (Caverject) are the only things that gave me acceptable results.

My 24 hour post opp update as follows:
Day 1.
Flew into Vienna 15/9 where after settling in at the hotel met with Professor Herwig at the one of the clinics they use for a CT Cavonosagram. The scan was easier than the one I had back in 1997 where it resembled more of a theatre procedure with my penis being pump up in a vacuum chamber then being flushed with dye to spot the origin of the leak before X rays were taken. Fast forward to 2015 I received an injection to force an erection then just stayed still whilst being pushed through the scanner so it could take colour 3D images of my pelvic area. After this was done Prof Herwig injected my penis with dye which spotted what he called a massive leak deep inside my penis. The scan lasted 10 minutes to which I then went off to explore Vienna.
Day 2
Arrived at the hospital at 8.45am, Dr Kuehass arrived at 8.55am where he promptly sat me down to go through the procedure including risks (mainly infection stuff). Told that the cavionosagram showed no evidence that my leaking veins travelled outside of the pelvic area which was a big relief that I wasn’t one of the 10-20% that do. Note to other that have had this procedure or are about to; I had the CT Cavonosagram because my case wasn’t a virgin venous leak, by virgin they mean just recently identified with no treatment offered thus far. Because I had my leak identified 18 years ago and received ligation surgery it was less cut and dry because veins had already been closed down so they needed to understand what was happening today and better gauge what my original surgeon had missed. The cost of this extra scan was about £200 but they wouldn’t have undertaken surgery without it in my case.
After the Dr K had finished I ended up waiting nearly two hours before I went into to have my surgery which although bored sitting in a waiting room I at least had the comfort the later than planned administered anaesthetic would provide more pain free travelling back to the UK.
When I was being prepped up in the theatre both Prof H and Dr K showed me the CT scan pics. To theirs and my surprise the leak was in fact coming from my deep dorsal vein which was supposed to have been removed 18 years ago. Personally I didn’t know what to make of this but both surgeons appeared very confident they could provide a successful outcome.
With regards to the surgery itself it was nothing too major, the most pain was the local anaesthetic being applied by needle although on a scale of 1-10 with 10 being max out pain this was only about a 4. Thereafter the pulling and tugging was a bit disconcerting whist holding your breath and pushing down to abdomen when they applied the agent was a bit of hard work on my part as they did this 3 times. The reason for doing this is so you apply pressure in the right area which prevents the scloriosing agent going anywhere else in the body. When finished I got the before and after X ray pics which clearly showed less blood leaking into my abdomen. Both surgeons were very happy and Dr K made me touch my penis to show that it was far more engorged in its flaccid state then before the vein was closed down. I was then bandage up and told to rest for an hour before leaving. Also given a prescription for daily 5mg Cialis; like Seeking and Razor although I wasn’t going to pay 100euros so went to see my GP this morning to get it on the NHS at £8.20.
Spent the afternoon in Vienna before departing for Heathrow at 8.30pm. I got back to my house tied but pain free at 1.15am in the morning!
Observations so far (after 24 hours post-surgery)
1. No pain yesterday or today.
2. Limited bruising around top of penis and testicles
3. Removed cock bandage this morning but still have the big plaster on that covers the incision which is stitched together with dissolvable stitching.
4. Felt some weak nocturnal erections whist lying awake wired from driving back. Tried to divert my thoughts from sex to other things as I didn’t really want an erection because of the fear of disrupting the wound and causing pain.
5. No morning wood albeit I didn’t sleep well last night whist my penis would I guess still be in shock from the surgery 12 hours before.
6. The head of my penis feels more engorged whilst flaccid this morning, however not sure yet whether this is swelling or not.
Hi Seeking – Was this pretty much the same for you? Maybe I’m paranoid but 24 hours post opp I feel relieved it’s over and more relieved I have no pain. This said I haven’t had the acid test yet which is can I get a harder longer lasting erection????
I guess this is it for now until I try and give it a run out over the next couple of days so I will look to update after the weekend.

No major risks to health apart from infection which although not pleasant can be treated with antibiotics.
Not a risk but I guess a reality with this type of surgery is it not working. More an emotional and financial hit really.
I'm confidently told it won't make your ED worse than it is (whatever that is?)

Thank you for the detailed report. I am also puzzled by the fact that the exam showed a leak coming from the deep dorsal vein when you had it ligated. Just conjecture here, but is that because the vein regenerates in some way? From what I understand, one of the main reasons that ligation surgery has such a poor outcome is that either new veins are grown, or the blood is redirected through other veins that were not removed. Honestly, what I've read is a little vague on that exact point. Is it that the specific vein that is removed somehow returns in the same place, or does the blood find other veins to travel back through? I'm a medical idiot so forgive my speculation. I've just never seen a really precise explanation for this. Anyway, I'll be eagerly checking in to see how you fare in the coming weeks. I have a Skype consultation with Kuehass on the 22nd but I'm not sure if it's even worth it. I want to ask him as specifically as possible and hear straight from him, just why this procedure is a significant improvement over the old ligation surgery.

I wish I could tell you, I really think the urologist worked with the X Rays and understanding back in the day and missed it. The reality is I was young and just wanted to be normal, I have no records from the '97 operation so the assumption that the deep dorsal vein was gone was made by Dr Kuehass on my first consultation. He was very surprised and pleased when it was still in tact and the current offending vein as it gave them more to work with.
