Welcome to the group, David. You have come to the right place. One of the best things you can do for your sexual health is to get into the best overall health possible, and with your medications it appears you are working on that side of things. Try finding a urologist who is very experienced in men's sexual health. I was referred to my urologist by my general practitioner, and as it turns out, she didn't know much more about men's sexual health than prescribing the usual ED pills.
Oddly enough, she is still my urologist and I actually may have benefited from her "let's go slow" approach. But if I had to start all over again I'd find somebody way more experienced in men's sexual health.
Unless your doctor has a problem with it, I would not be overly concerned about having a few alcoholic beverages. There are some cures for ED that will work for people who are social drinkers.
We all have to accept some tradeoffs as we get older. I am 60 and also produce less semen than I did as a teen. I think that is natural. Your urologist can discuss the best solution for you to get and maintain a very good erection. I tried all the pills but did not have any satisfying success until I began injecting a drug called Trimix directly into the penis before sex. Once you get used to the idea it doesn't hurt at all and the erections are phenomenal. However, there are some possible drawbacks, and you should read up on Trimix thoroughly before discussing with your doctor. This board is a good place to start.
Some guys move all the way up to penile implants, and there is plenty of discussion here about that too. The bottom line is that there definitely solutions for ED. Nothing can make you 18 again, but with a little help from the medical community you can certainly have sex like an 18-year-old once more.
